Because of privacy concerns and in order to locate your records you will be asked to verify some of your personal information. Some of the information you may need to have available is:

•Full name of the priesthood holder
•Date of birth
•Current ward and stake
•Current priesthood office held
•Address and ZIP Code
•Who performed the ordination
Member Services will mail or fax (if you request it) the record to you. Currently, the church is not offering to send your priesthood line by email.
